بررسی فرآیند حمل و جابجایی بیمار بین تخت و برانکارد و مشکلات ناشی از آن در محیطهای بیمارستانی
A Survey of Patient Handling between Bed and Stretcher and Associated Problems in Hospital Environments

<p dir="RTL" style="text-align: justify;"><strong>مقدمه: </strong>اختلالات اسکلتی- عضلانی از عوامل اصلی ناتوانی­های شغلی در کارکنان مراقبتهای بهداشتی است که حمل و جابجایی بیمار از دلایل مهم آن است. هدف این مطالعه شناسایی شرایط خطرناک مرتبط با حمل و جابجایی افراد بستری در بیمارستان­ها بود.</p>
<p dir="RTL" style="text-align: justify;"><strong>مواد و روش­ها: </strong>در این مطالعه مقطعی، اطلاعات مربوط به فعالیت­های جابجایی بیمار در 23 بخش از سه بیمارستان بزرگ شیراز با استفاده از پرسشنامه پژوهشگر ساخت بهصورت مصاحبه با حداقل 23 نفر گردآوری شدند. تعداد جابجایی­های دستی در شیفت کاری، میزان توانایی بیمار برای جابجا شدن، آسیب نواحی مختلف بدن پرسنل و ضرورت وجود وسایل کمکی مکانیکی مورد بررسی قرار گرفتند.</p>
<p dir="RTL" style="text-align: justify;"><strong>یافته­ها:</strong> جابجاییهای دستی بیمار در 70% از بخش­ها بیشتر از 5 بار در یک شیفت کاری بوده و در بیش از 70 درصد از بخشها بیماران "خیلی کم" و یا "اصلاً" قادر به همکاری جهت انتقال نبودند. در بیش از 80% بخش­ها پرسنل دارای وظیفه جابجایی بیمار دچار آسیب نواحی مختلف بدن بوده که شایعترین آن کمردرد (78/48%) بود. وجود وسایل کمکی مکانیکی توسط اکثر (95%) آزمودنی­ها ضروری و مفید دانسته شده و به نفع پرسنل، بیمار و بیمارستان قلمداد گردید<span dir="LTR">.</span></p>
<p dir="RTL" style="text-align: justify;"><strong>نتیجه­گیری:</strong> در اکثر بخش­ها بیماران نیازمند کمک دیگران جهت انتقال به تخت و یا برانکارد هستند. اکثر این جابجایی­ها بهصورت دستی و به کمک پرسنل درمانی انجام می­گیرد. شیوع ابتلا به آسیب­های اسکلتی- عضلانی در اندام­های مختلف بدن پرسنل مسئول بالا بوده و اجرای اقدامات ارگونومیک مانند استفاده از وسایل کمکی در این زمینه ضروری است.</p>
<p style="text-align: justify;"><strong>Introduction: </strong>Work-related musculoskeletal disorders (WMSDs) are among the major causes of work-related disability in healthcare workers (HCW). One of the main causes of WDSDs is patient handling. This study aimed to identify hazardous conditions related to patient handling in hospital environments.</p>
<p style="text-align: justify;"><strong>Material and </strong><strong>Methods: </strong>Patient handling activities were evaluated in 23 wards of three hospitals in Shiraz, Iran. A researcher-made questionnaire was used to collect the required data through interviews with at least 23 HCW. Number of patient transfers per shift, patients’ ability to move to/from bed/stretcher, type and rate of personnel injuries, and necessity of transfer assist devices were surveyed.</p>
<p style="text-align: justify;"><strong>Results: </strong>Over five manual patient transfers per shift were performed in 70% of the wards. More than 70% of the patients were completely or relatively unable to cooperate in their own transfer. In more than 80% of the wards, the staff involved in patient handling activities suffered from various WMSDs, mainly low back pain (48.78%). Almost all (95%) participants believed that transfer assist devices were necessary and helpful and could be beneficial to not only the patients, but also the HCW and the hospital.</p>
<p style="text-align: justify;"><strong>Conclusion: </strong>Patients in most hospital wards need help when moving to/from bed/stretcher. Patient handling is generally performed manually by HCW. The high prevalence rate of WMSDs in the participating HCW highlighted the necessity of an ergonomic intervention, e.g. the use of transfer assist devices, for patient handling.</p>
